Well-Child Visits and Immunizations
It’s still important for children to see their health care providers for any health concerns, and to schedule their children’s well-child visits.
There have been no changes to the California State Immunization requirements for the 2022-223 and 2023-2024 school year. All children will need to meet all immunization requirements by the first day of school in the fall. Any updates to California immunization requirements for schools will be posted here.
Immunization Requirements for 2022-2023 and 2023-2024 School Year
All TK/Kindergarten:
- Physical Health Assessment (CHDP) Form - physician’s office can provide
- 4-5 year old vaccinations
- 4 Polio
- 5 Dtap
- 2 MMR
- 3 Hep B
- 2 Varicella
All students entering 7th grade:
- Tdap vaccine given on or after the 7th birthday
- 2 varicella vaccines
Medication
All students who will need to take medication during school hours will need a medication form completed by their HCP and signed by both the parent and provider. Please note medication is to be provided by the parent in the original container. Additionally, if a student will need an inhaler, parents are asked to please include a spacer/aerochamber to minimize respiratory droplets in the air.
Medical Forms
If a student has a seizure disorder or food allergy, parents are asked to please have their child’s health care provider fill out the required forms. These forms need to be updated every school year.
For your convenience, you may use this Immunization Upload Form (22/23) and Immunization Upload Form (23/24) to upload vaccination records. You may also bring hard copies to your school office M-F 9am-2pm.
